$10 per application. Refundable after completing 2 hours of volunteer time. |

Refund Policy:
To cancel or request a refund, a written notice must be post marked by August 31, 2010 to : LYSA Refunds, PO Box 1031, Londonderry, NH 03053.
Refund Schedule based on postmark date:
Before July 15 – 100%
July 15 and July 31 – 80%
August 1 and August 14 – 70%
August 15 and August 31 – 60%
After August 31 – 0%
